Case Management: Linking you to community resources and helping you coordinate your care 

When an individual struggles to understand and take advantage of the community resources offered to them, a case manager can help. At Access Living, we offer case management services to help individuals and families identify and coordinate the use of available resources to help them have more stability and ensure their basic needs are met. Services may include facilitating access to medical resources, vocational/educational options, financial assistance, legal assistance, food assistance, and assistance with housing. 

Case management is covered by Medicaid or can be utilized for a cash pay price of $60 per hour.
